Our Behavioral/ADHD Diagnosis Packet includes the following important information and forms:

Our Behavioral Health Policy which outlines the financial arrangements and obligations of patients and their insurance company.

The Behavioral Initial Visit form which asks you to review the child’s behavior in various settings and is to be completed by the parents prior to the initial visit.

The Vanderbilt Parent Questionnaire which needs to be completed by the parents. We recommend that each parent complete an individual form.

The Vanderbilt Teacher Questionnaire. Please print and ask your child’s teacher(s) to complete the Vanderbilt Teacher Questionnaire. In addition to your child’s present teacher, we recommend that you have as many past teachers as possible also complete the questionnaire.
