School Health Forms
We are happy to complete required forms for school, camp, athletics, etc. The charge is $10 for most forms.
If your child has not had a physical exam within the previous year, please schedule a 10 minute appointment so we can complete the form with current information.
School Entry Form
California law requires a health examination for school entry. Complete the parent/guardian portion of this form and bring it to our office for completion.
