Behavior/ADHD Evaluation & Diagnosis
If you would like your child to be evaluated for ADHD or other behavior-related concerns, please print and complete the appropriate forms and bring them to our office. After we have received all of the forms, your doctor will review them and we will call you to schedule an appointment to go over the information and formulate a plan.
