Evaluation & Diagnosis
Behavioral, emotional, and educational issues are typically complex with many contributing factors. Identifying all factors involved is often a complicated process and therefore we feel it is best to have your child evaluated by a specialist in that area.
Options for evaluation include:
- Upon your request, your school district can complete an evaluation.
- You can call your insurance for a list of providers who are on your plan.
- You may call our office to obtain information on providers we recommend.
Autism Screening Form
This questionnaire is used to help identify children who are at risk for autism. Please print, complete, and bring to your child’s 18 month and 2 year visits. You may also use this form for any child under the age of 5 years.

Visit Dr. Peter and Michelle Contini virtually through Almaden Pediatrics’s new telemedicine portal. It’s quick, secure, and covered by most insurance plans.
To start an online visit, click on "Start Now". First time users will need to create a free HealthLens account, which takes about 5 minutes. Then describe the condition, answer a few questions, and upload 1-3 sharply focused photos of the condition. You will also need to upload a recent “selfie” with the condition photos.
Dr. Contini will evaluate the condition and return a diagnosis and treatment plan in less than 48 hours. If the treatment plan includes a prescription, it will be sent directly to your preferred pharmacy.
Currently, most Blue Cross, Blue Shield, Cigna, United Healthcare, and Aetna plans cover this service. More insurance plans are starting to cover telemedicine visits, so check with your carrier.
If you are the parent or legal guardian of the patient, you may make an account on their behalf and submit visits for them. Each patient must have their own account. You can use the same email for multiple accounts.
